Records

What this manual covers.

  • Company registration and governance records.
  • Public website, policy, consent, security, and operational records.
  • Supplier, partner, professional, payment, support, and service-provider records.
  • Patient, assessment, consent, care-support, and portal records where access is lawful, identity is verified, and disclosure is not refused under PAIA or another law.
  • Records required by POPIA, ECTA, tax, employment, health-service, consumer, and other applicable South African law.

Requests

How to ask for a record.

  • Send a written request to privacy@cendara.co.za and identify the record as clearly as possible.
  • State the right you want to exercise or protect and why the record is needed for that purpose.
  • Say whether you are asking for yourself, for an authorised person, or as another lawful requester.
  • Include enough information for Cendara to verify identity or authority before any sensitive record is disclosed.
  • Keep urgent medical concerns, lab values, payment secrets, and unnecessary health detail out of email.

Clinical partner

Mobi-Med Clinic review is handled carefully.

Cendara coordinates online weight-care review with Mobi-Med Clinic. Records connected to clinical review, consent, prescriptions, lab context, pharmacy coordination, or portal support may require extra verification before disclosure.

Limits

Access is not automatic.

  • Cendara may refuse or limit access where PAIA protects another person's privacy, confidential commercial information, safety, privileged material, security, or records that are not required for the exercise or protection of a right.
  • Cendara may separate, redact, or withhold parts of a record where only part of the record may lawfully be disclosed.
  • Cendara may charge a prescribed request or access fee where the law allows it.
